Proof of Residence

• Utility service contract, bills or payment receipts (Gas, Water, or Electricity)
• Property taxes, rental or lease agreement, current rental receipt with address of property on receipt
• Official government mail (CalWORKS, Social Security, Medi-Cal)
• Current pay stub
• Voter registration

Proof of Age of Minor

• Birth certificate
• Baptismal Certificate
• Passport
• Court order
• Health office/vital statistics record of birth certificate date
• Department of Public Social Services (DPSS) letter verifying birthdate and an explanation of how this was verified.

 Parent/Guardian/ Educational Rights Holder/ Caregiver ("parent") Identification

• Student’s birth certificate or Court order establishing the parent/guardian relationship AND
• Adult’s government-issued photo identification (Driver’s license or DMV identification card)

Proof of Immunization

If available/applicable, you may also submit copies of:

• Transcripts or recent grade reports
• Copy of most recent Individual Educational Plan (IEP) or Section 504 Plan
• Department of Children and Family Services (DCFS) 1399 Form or minute order
• Department of Probation placement information and/or Juvenile Court minute order
• Court orders regarding legal custody, physical custody, or restricted contact with the school or child.

Comprobante de Edad del Menor

• Acta de nacimiento
• Acta de Bautismo
• Pasaporte
• Orden judicial
• Registro de la oficina de salud/estadísticas vitales con la fecha del acta de nacimiento
• Carta del Departamento de Servicios Sociales Públicos (DPSS) verificando la fecha de nacimiento y una explicación de cómo se verificó

Identificación del Padre/Tutor/Titular de Derechos Educativos/Cuidador ("padre")

• Acta de nacimiento del estudiante o orden judicial que establezca la relación padre/tutor Y
• Identificación con foto emitida por el gobierno del adulto (Licencia de conducir o tarjeta de identificación del DMV)

Si está disponible/aplicable, también puede enviar copias de:

• Transcripciones o reportes de calificaciones recientes
• Copia del Plan Educativo Individualizado (IEP) más reciente o Plan de la Sección 504
• Formulario 1399 del Departamento de Servicios para Niños y Familias (DCFS) o acta judicial
• Información de colocación del Departamento de Libertad Condicional y/o acta judicial del Tribunal de Menores
• Órdenes judiciales relacionadas con la custodia legal, custodia física o contacto restringido con la escuela o el niño
